What to Expect at Your First Visit
We want your first visit to Primary Health & Wellness to feel comfortable, simple, and informative. Our team will take time to discuss your symptoms, health history, lifestyle, and treatment goals before completing an evaluation and recommending a personalized care plan. Please complete any required forms in advance and bring your identification, insurance information, and relevant medical records to your Houston or Humble appointment.
New Patient Forms
New patients must complete a New Patient Form in either English or Spanish. You can complete and print the form before your appointment or fill it out when you arrive.
Please use a pen or pencil to mark the location of your pain on the form. We offer flexible scheduling, including evening and weekend appointments.
